Does Nokia make changes to updates between number changes?  Case in point...if I install v31 FW to my phone and then reinstall v31 a couple of months later, do I get the same exact FW or are there minor updates included in the FW?

Generally not. The numbers after the first full stop refer to regional, product code and other differences. All you compare is the first set. So if you already have v31 from 3 months ago then it is still the same as v31 today.

The one reason I asked is that, before I re-installed the FW there were no silhouettes in the call screen (calling and receiving call from someone without an assigned picture).  After, I now have a silhouette in the call screen.  Weird, but a nice touch.

From what I understand so anyone please feel free to correct, the firmware number is in 3 parts separated by fullstops like xxx.x.xxx. The first part is the version number, the second part refers to region and the last part refers to a minor update. I have yet to notice any difference myself after installing a minor update.

The changes in your call screen may be due to an added feature or it could also be due to a changed setting resulting from the update. Only Nokia can confirm this.
